Include the following info:
1. Baby's name (full name and correct spelling)
2. Special quote or meaning of name(s)
3. Preferred colors to be used (three or four at most)
4. Baby's birthdate
5. Baby's birth weight and birth length
6. Time of birth
7. City and State of birth
8. Picture of newborn (optional)
9. Color of mat (if purchasing with mat)
